Meeting The Challenge
In this challenge you will add a form to the Home page of your client's website that allows users to submit a request for additional information.

1. Create a form using at least three different form objects to collect a first name, last name, street address, city, state, zip code, and type of information requested.
2. Include clear instructions that will help users fill out the form correctly.
3. Add text labels to each form object.
4. Add a Submit and Reset button and format them appropriately.
5. Format the form with a CSS.
6. Preview the page and test both the Reset and Submit buttons.
7. Edit the form as necessary.
8. Use the rubric found on the Knowing You Have succeeded page to self-assess your work. When you are satisfied you have met all of the requirements, find this assignment in your Google Classroom and select TURN IN. Contact your mentor using the Subject Line "Request for Help" if you need assistance.
